HEALTHCARE PROGRAM «MY HEALTH» FOR ADULTS FROM 15 TO 55 YEARS OF AGE

The program addresses all health care needs for adults aged between 15 to 55. The services are provided by the attending family practice doctor at the clinic and by a duty doctors at home. Routine check-up, specialist’s consultations, procedures, lab tests will be performed at the clinic on referral of the attending doctor.

This program covers the services provided only by EMC doctors.

Out-patient services & house-call visits

• Attending doctor: routine check-up and treatment at the clinic. The doctor is available at the mobile phone through his assistant or by e-mail: weekdays from 10.00 to 20.00. After 20:00, on the weekend and holydays, please, contact duty doctors (24h);
• Specialized out-patient care if indicated: General therapy, allergology & immunology, gynecology, gastroenterology, nephrology, urology, pulmonology, hematology, dermatology, neurology, cardiology, ENT, ophthalmology, traumatology orthopedics, minor surgery, proctology, endocrinology;
• Diagnostics&Laboratory: Functional diagnostics, x-ray, CT scan, MRI, ultrasonic examination, endoscopy, clinical laboratory, hematology, cytology, histology, biochemistry, hormones, coagulation, immunology, bacteriology, microchemistry, microbiology, oncology;
• Rehabilitation: 10 sessions of each therapy per year if indicated. Physiotherapy, medical massage, therapeutic exercises, acupuncture;
• Vaccination: All kinds of vaccination, different vaccines on patient’s choice;
• Medical certificates: Medical reports, certificates (Russian, English, French, Japanese) except certificates for migration services (CMIGR);
• House-call visit (provided within 50 km outside the Moscow ring road. Additional cost is applied for house call out of Moscow, see. «Costs chart»):Consultations, procedures and blood sampling, functional diagnostics, emergency & transportation in case of acute condition. House-call visits are provided by duty doctors 24h.

Dental care

• Therapy: Primary dental check-up provided by therapists, orthopedics, surgeons, periodontist, diagnostics & treatment. Diagnostic & therapy in case of acute or worsening of chronic dental diseases (sealing and treatment of root canals by caries or its acute complications);
• Dental surgery: Dental surgery by acute or worsening of chronic dental diseases (teeth extraction, treatment of acute inflammations);
• Hygiene: Scaling, superficial fluoridation once per year;
• X-ray: Radiovisiography, x-ray (including panoramic).

Emergency and scheduled hospitalization.

Emergency and scheduled in-patient stay and treatment is provided strictly under medical indications in case of: Gynecology, gastroenterology, neurology, cardiology, otoryngology, ophthalmology, traumatology and orthopedics, (general) therapy, surgery, coloproctology, neurosurgery (vertebrology), urology, maxillo-facial surgery, endocrinology.

In-patient stay length and medical expenses’ limits*: Medical expenses – less than 4 000 per year.

*-hospitalization expenses = hospitalization days+ surgical team manipulations + surgery cost + anesthesia cost

The program does not cover treatments for the following ailments:

• Disability, radiation disease;
• HIV;
• Pregnancy;
• Dangerous contagious diseases: plague, cholera, smallpox, yellow fever, anthrax, epidemic typhus and other;
• Diabetes I & II types;
• Psoriasis;
• Psychiatry, epilepsy, mental disorders and their complications, conditions or diseases of mental origin;
• Alcoholism, drug addiction;
• Malign tumors and formations including blood oncology;
• Tuberculosis, sarcoidosis and their complications;
• Congenital abnormalities (malformations), deformations and chromosomal abnormalities; infantile cerebral paralysis;
• System affection of connective tissue, demyelinating diseases of neural system;
• Pregnancy;
• Sexually transmitted diseases, including ureaplasmosis, gardnerella, mycoplasmosis, HPV;
• Chronic kidney or hepatic insufficiency requiring hemodialysis;
• Viral hepatitis, liver cirrhosis.

The program does not include the following medical services:

• Genetic investigations;
• Psychiatric/psychologic services;
• Expensive medications, glasses, lenses, hearing devices or implants, transplants, prosthesis, endoprosthesis, pacemakers, orthopedic devices, adjustment of compensating medical devices;
• Cosmetic dentistry, orthodontics, prosthesis, implantology and preparation before implantology or prosthesis, sealing with thermofils or hot gutta-percha, surgical treatment of periodentium, extraction of retained and distopic teeth, dental CT scan, deep fluoridation;
• Medical and surgical correction of vision including LASIC and orthoptic treatment;
• Coronarography, angioplasty, stenting;
• Surgical hemodialysis;
• Trasplantations;
• All kinds of plastic surgery;
• Infertility diagnostic & treatment, IVF, contraception, impotence treatment;
• Abortion without medical indication; maternity;
• Aesthetic procedures and surgery, cosmetology including weight correction, sex reassignment surgery, sclerotherapy;
• Check-up for migration service (CMIGR).
