Andrology, (andros – male, and logos – science, studies), studies the male health. As a medical discipline, andrology deals with diagnosis and treatment of male sexual sphere disorders. There are diseases associated with pathologic function of male sex organs, such as, for instance, inadequate erection. Most often, we are talking about weak erection or its complete absence. Also, andrology studies the problem of testicle dysfunction. During certain organism conditions, these hormonal glands are incapable for production of required amounts of corresponding hormone. This dysfunction can bring about incorrect intrauterine fetus development. A harmless at the first glance testicle dysfunction is the cause of development of such pathologic conditions like hermaphroditism, or can bring about mastopathy and sterility in “healthy men”. Finally, an integral part of andrology examines the cosmetic issues, such as congenital defects, small (or excessively large) penis, absence of testicles, etc. We should note the important social role that andrology plays in nowadays world. In the era of technical progress, all sciences are actively developed, inclusive of medical sciences. In its turn, during last decade andrology made an incredible leap forward. First of all, this is associated with high level of development of humanity, with all that it implies actual needs to comply with the highest andrologic standards. Secondly, using new technologies, such as prostheses and their components, newest medicinal drugs or cell and genetic engineering, allows considering andrology as one of the front line and high-tech medical sciences. In the present days medicine laparoscopy is used widely. In urology, this method steadily occupies an increasingly significant share of other surgical operations. Laparoscopic surgery is carried out not only for varicocele and renal cysts, but also during nephroptosis (descendent kidney), stricture (narrowing) of ureteropelvic segment, kidney and ureter stones, and during prostate and kidneys neoplasms. The benefits of laparoscopic operations are beyond doubt. However, it should be noted that in exceptional cases the open abdominal surgery can not be avoided. We are compelled to take such decision due to some particular features, inclusive of individual body anatomy. The complications during laparoscopic operations are the same as during abdominal surgery. However, their degree is minimized.
With the exception of very rare cases, and due to absolute contraindications, the operations for kidney cyst removal are carried out at EMC using laparoscopic technique. The post-operation period is rather short, and in 2-3 days the patient can resume customary way of life. Nephroptosis (prolapse of the kidney, wandering kidney) After careful examination and setting the indications for operation, we carry out laparoscopic nephropexy (or suspension of movable kidney) using special polypropylene mesh. The first laparoscopic nephropexy in Russia was carried out by Dr F.V. Shakir in collaboration with French colleagues, Drs P. Monaud and C. Defey. After laparoscopic operation, it is not necessary to stay in bed for a long time (which, in case of open abdominal surgery, can be as long as 10 days), and in a few days the patient is discharged from the clinic. Stricture (narrowing) of ureteropelvic segment The laparoscopic plastic surgery of ureteropelvic segment is carried out with purpose of reconstitution of urine outflow at the kidney level. The rehabilitation time with these operations is much shorter as compared to abdominal surgery. The removal of kidney and renal duct stones at the EMC is carried out using laparoscopic technique. This is the most complex laparoscopic operation. In Moscow, this operation is carried out only by EMC. During laparoscopic prostatectomy, like during abdominal surgery, the prostate along with semen vesicles is removed. After resection, the prostate along with semen vesicles is placed in a special polyethylene bag, which allows their removal without extended skin cuts. The drainage of post-operation wound and bladder is much less lengthy as compared to abdominal surgery, and the recovery period also passes much faster. To carry out this operation, we invite the French urologist team, professors P. Monaud and C. Defey, who have an immense experience in performing this operation. Urethral catheter is placed for approximately up to 7 days. After abdominal surgery the bladder catheter placement duration is 21 days. |
