The EMC otorinolaryngology department provides the full spectrum of treatments for ear, nose and throat including prevention, diagnostics, therapy and surgery.
Our doctors perform the full spectrum of operation with a focus on endoscopic surgery: adenoidectomy, septoplasty , surgery of the accessory sinuses of nose, tonsillectomy, shunting of the tympanic cavity, surgery of otitis media chronica, surgical hearing correction and restoration, and surgery of the vocal organs. The main advantages of endoscopy include high accuracy, minimal risk of post surgery complications and fast recovery.
Our ENT staff widely use the latest diagnostic methods:
- X-ray;
- Computer tomography;
- MR-imaging.
This enables quick and proper diagnostics and treatment.
Our otorhinolaryngologists have had many years experience in the leading European and Russian clinics, and provide all types of conservative and surgical treatment for children and adults.
Emergency operations are done round-the-clock.
As separate working areas we mark out audiology– hearing testing, deafness and hearing loss examination– and hearing aids – hearing improvement surgery, including implant surgery.
Moreover, our ENT department deals with the following problems: imbalance, sleep apnea (respiratory standstill in sleep), and snoring using different techniques (ссылка на описание методик).
Audiology
EMC hearing rehabilitation department provides medical assistance to children and grownups.
Hearing loss can occur at every age. Early detection of hearing loss can prevent nervous system and communication distortion, as well as problems with social adaptation.
A child’s hearing loss is successfully identified soon after birth. Hearing is a prerequisite for successful speech development and intellectual growth of a child. Children suffering fr om significant hearing loss cannot develop their life potential without special assistance. That is why early identification of hearing loss and its treatment is essential for child’s adequate development.
Thanks to multi-field medical staff – otorhinolaryngologists, surgeons, audiologists – and latest diagnostics equipment we can identify hearing loss of different degrees.
In the EMC we do following subjective and objective tests:
- Pure-tone audiometry;
- Otoacoustic emission;
- Impedancemetry;
- Multilingual speech audiometry for children (Pilot-test).
Each objective acumetry technique guarantees limited precision, and only complex investigation using diverse acumetry techniques can almost exclude diagnostic errors.
If hearing loss is detected, the EMC audiologist develop an individual hearing restoration plan. Combination of treatment and correction methods: therapy, surgery, traditional hearing aid, hearing implants stands for explicitly good results even in the most complicated cases.
Hearing aid surgery
Hearing aid surgery is considered as most complicated operations in otorhinolaryngology. In the EMC we install different implants, including:
- bone conduction implants Cochlear BAHA in case of inherited hearing loss, otitis and hemianacusia;
- middle ear implant system Vibrant Soundbridge Med-El (ссылка на описание) and Otologics LLC are used in case of sensoneural hearing loss;
In November 2008, our otorhinolaryngologist Jean-Rene Millier installed a Vibrant Med-El hearing implant for the first time in Moscow.
Bone-anchored implants
Inborn or evoked hearing loss is a serious proplem, impeding the normal way of life. Only few know that sound perception significantly depends on chest bones. The sound conductivity of the bones can be used for hearing restoration in case of significant hearing loss.
How do we hear?
Everybody knows historical fact that Ludwig van Beethoven who suffered from progressive hearing loss continued to compose music. But how could he do it? He took an item and put it at his head and the other end of this item he leaned against the musical instrument. This way he could hear better.
If you increasing press an item with sound vibrations against your chest bone, its sound conductivity significantly increases. This technique came into use in otorhinolaryngology for hearing restoration. All Cochlear Baha hearing implants have been developed based on bone conductivity and are installed directly into the bone tissue.
For how long such implants have been used?
First attempts to implant a hearing aid started 1977 in Sweden. Dentists and maxillofacial surgeons implanted diverse titanium elements into a patient and noticed an improvement of hearing. Afterwards this unexpected result was used for hearing loss treatment.
How does the implant function?
This device has quite a specific construction. It consists of a titanium implant. It is connected to an external abutment, and a sound processor that transmits sounds to the chest bone.
It is interesting that this device is surgically implanted from the damaged side and sends a signal to the other healthy ear wh ere the sound is processed. But the brain perceives the sound as if the person hears from the both sides.
Who is a candidate for the Baha system?
The Baha is used for rehabilitation of people with total hearing loss (one-sided or both-sided) caused by middle and inner ear infections: chronic otitis, congenital anomaly, postsurgical complications and after effects of illnesses. This device is a proper solution for those who cannot use cochlear (endaural) hearing aids. Even in case of complete loss of the outer pinna the Baha enable the Patient to hear.
How is the device installed?
The titanium implant and the external abutment are implanted during one operation with local anesthetic. The sound processor is put on in a month or month and a half depending on the patient’s health status. This device does not prevent sporting activities and normal life, but it is recommended to take off the sound processor when swimming and in hydrotherapy. You can do this quite easily!
What is the operation time?
The operation takes 1-1.5 hours and is done with local anaesthetic. In the evening on the day of surgery the patient is discharged from the clinic.
Is the Baha implant visible?
The Baha is implanted behind the pinna of the damaged ear. The colour of the sound processor can be selected in accordance with hair colour so the device is almost unnoticeable.
What is a middle ear implant?
The Vibrant Soundbridge Med-El middle ear implant system mechanically causes the middle ear ossicles to vibrate. This device consists of two elements: internal implant and external sound processor. Middle ear implant systems are used to rehabilitate people with high-frequency hearing loss, conductive and mixed hearing loss (otosclerosis, external and middle ear maldevelopment, consequences of the chronic otitis, etc.). The Vibrant Soundbridge implant system has significant advantages for patients who for some reason cannot wear hearing aids or have had bad experience with hearing aid usage.
Snoring
For many men and women, snoring is a social problem that can hardly be resolved without assistance. The snoring is mostly caused by vibration of the soft palate. In sleep, soft tissues of the palate become relaxed ,start to vibrate spontaneously and cause unpleasant sounds. The reasons for snoring can vary, for example particular anatomic organization, nasal cavity deformation and poor ventilation, lower jaw maldevelopment, malocclusion, excess weight, lack of sleep, tiredness, smoking, alcohol etc. On the other hand, the snoring can be a sign of serious diseases that require medical treatment. The most dangerous snoring type is apnea, sudden and long respiratory standstill in sleep.
Dr. J.R. Milliet, the French otolaryngologist and surgeon provides the treatment for snoring through:
Diathermy
Diathermy is a method that uses electric current of high frequency to strengthen weak palate walls, reducing their vibrations in sleep. The diathermy technique is successfully used for snoring treatment in Western Europe. This method is painless and efficient in 91% of cases (in other cases, surgical intervention can be recommended). Diathermy of the soft palate is done during consultation with local anesthesia. In a minute Dr J.R. Milliet makes three short sessions of 20 seconds each. The efficiency of this method can be evident in a few weeks: this the time required for structural changes of the soft palate tissues.
Jaw splint (orthosis) puts the lower jaw a little forward which results in increased lumen of the larynx and eliminates snoring. The patient is recommended to wear the orthosis at night and take it off in the morning.Diathermy of the soft palate is done during consultation with local anesthesia. In a minute Dr J.R. Jaw splint
In our clinic, we use a last generation orthosis O.R.M.® Device made by Laboratoires Narval. This device is produced individually for each patient. The O.R.M.® Device was clinically tested on 108 patients in Western Europe and with the following results:
- snoring reduction by 80%;
- elimination of short respiratory standstill in sleep by up to 60%.
Moreover, the orthosis is very easy in use: it is invisible, thinner and lighter in comparison to many similar jaw splints, does not lie heavy on teeth, and as result does not affect the speech. Today, it is the most efficient and convenient technique for non-surgical treatment of snoring.
